A key part of our representation role for Primary Care is a partner in the Hillingdon Health and Care Partners (HHCP). This is a partnership with a formal written agreement that includes The Hillingdon Hospitals NHS Foundation Trust (https://www.thh.nhs.uk/), Central and North West London NHS Foundation Trust (https://www.cnwl.nhs.uk/), and H4All (https://www.h4all.org.uk/) a Charitable Incorporated Organisation (CIO) made up of five prominent third sector charities: Age UK Hillingdon, DASH, Hillingdon Carers, Harlington Hospice and Hillingdon Mind. This partnership is formed as an integrated care partnership that seeks, through collaboration and co-design, to make significant improvements to the quality and cost of care in Hillingdon. By providing an equal voice for primary care in the partnership we make sure the views and experience of GPs and their teams are represented. This also recognises the fundamental role of General Practice in being the first point of contact for most patients, most of the time; and seeing more patients in total than all other parts of the NHS combined.
We also maintain key relationships with the London Borough of Hillingdon Council, Brunel University, the NHS Hillingdon Clinical Commissioning Group and various NHS and research and education bodies particularly in NW London.
